A teenager from the Saegertown area died Wednesday when a forklift he was operating rolled over onto him.

Investigators say 17 year-old Jacob Byler was backing the forklift at an industrial company in the Cochranton area when a rear wheel caught itself in something on a loading dock.  When Byler freed the machine, it toppled and fell on top of him.

Byler was pronounced dead at the scene.

An official from the Occupational Safety and Health Administration (OSHA) will investigate.
